Trinity Care Navigation was founded in January 2025 with a clear mission: to deliver personalized, clinically guided care navigation solutions that improve health outcomes, reduce costs, and enhance the member experience. While the company itself is newly established, our leadership has been administering Care Navigation Programs since 2015, bringing nearly a decade of hands-on experience to the table. We’ve worked extensively with self-insured employers across various industries, providing both the clinical and administrative insight needed to drive measurable results.

Dedicated Patient Navigator Team
Each member is supported by a specialized team, including nurses and trained non-clinical staff, who guide them through the healthcare system, helping them access necessary services, schedule appointments, and coordinate care effectively. We reduce unnecessary ER visits, specialist referrals, and duplicate testing by guiding members to the most appropriate and cost-effective care.
